We offer a range of clinics and services at the surgery. There are some tasks you can do, such as health reviews, using our Accurx service.
The nursing team
The nursing team at The Chorley Surgery consists of a nurse prescriber, practice nurse and a healthcare assistant. Take a look below at the range of services our nursing team covers.
Sexual health and cytology
The practice nurses can review contraception including medication and administer Depo-Provera contraceptive injections. Take a look at the link below to decide which contraception method is best for you.
Appointments for cervical screening (smear tests) are available for women aged 25 to 49 who receive invitations every 3 years. Women aged 50 to 64 receive invitations every 5 years and you must have a reminder letter before you book the appointment. Please note the smear test cannot be performed if you are on your period. The link below explains why the smear tests are done and what happens at the appointment.
Health promotion
The team give lifestyle information on modifiable factors such as stopping smoking, losing weight, increasing exercise or healthy eating.
Treatment room
Our treatment room services include:-
- Basic dressings/wound care
- B12 injections
- ECG’s
- Urine testing
- Blood Pressure monitoring
Chronic diseases
These include Type 1 and Type 2 Diabetes, Heart Disease and Stroke. We also run respiratory clinics for those with Asthma or COPD (bronchitis or emphysema) and Spirometry (lung function testing).
Immunisations
We administer immunisations as part of the childhood vaccination programme and as routine for adults such as pneumococcal, shingles and seasonal flu immunisations. We also run travel clinics, advising and vaccinating individuals travelling abroad. If you’re unsure of your baby’s immunisations these can be found in your red book, or click on the link below.

Seasonal flu vaccination
Every Autumn the practice runs a programme to vaccinate those at risk from influenza e.g. the over 65’s or anyone with chronic illness, heart disease, hypertension, diabetes, lung or kidney disease or asthma. If you’re unsure if the flu vaccination is appropriate for you, ask a member of the team and we will be happy to advise. The injections are given at our flu clinics throughout September, October and November and are free of charge.
Phlebotomy clinics
The blood clinic which runs at Chorley Hospital runs Monday to Friday from 8:30am to 4:30pm. You can just turn up here and take a ticket. The blood clinic which runs from Collison Avenue is pre-bookable. The clinics here run on a Monday and a Friday from 8:30am to midday, these are 5-minute time slots and can be booked by the receptionist here at The Chorley Surgery.
Annual reviews
Practice nurses and our healthcare assistant undertake a variety of annual reviews to help manage long term conditions. If you have a chronic condition you will be called in for your annual review.
NHS health checks
NHS health checks aim to prevent heart disease, stroke, diabetes, kidney disease and certain types of dementia and are targeted at 40 and 74 year olds.
On the day of your NHS health check:
The NHS health check is carried out by a healthcare professional. This will usually be our healthcare assistant. The check takes about 20 to 30 minutes.
You’ll be asked some simple questions, including:
- family history
- if and how much you smoke
- how much alcohol you drink, if any
- what your diet is like
- how much physical activity you do
Your weight and height will be measured to work out your body mass index (BMI).
Your waist may also be measured using a tape measure.
Your age, gender and ethnicity will be noted.
Your blood pressure will be taken using a cuff fitted over your upper arm.
Also, a urine sample is needed.
If you are 40 years of age or over a blood sample should be taken before your health check to check your cholesterol and for diabetes.
This health check can be booked over the phone or at reception.
Medication reviews
The purpose of a medication review is to check that your medications are the best ones for you. It involves discussing your medicines with you and checking if they are working without causing any unwanted side effects. It also gives you a chance to tell us how you are getting on with them and ask any questions to find out more about them. Medication reviews should be done annually or sooner depending on your medical condition. Medication reviews can be done face to face or over the telephone depending on the medication you are taking. These are done with our in-house pharmacist and can be booked over the phone or at reception.
Travel clinic
Travel health advice & travel immunisation service – With increasing numbers of people travelling abroad, we recommended that you seek advice from our practice nurses regarding the appropriate vaccinations for your destination. Please plan well ahead and book your appointment at least six weeks before you travel.
Maternity services
We have a midwife run clinic which runs every Thursday afternoon from The Chorley Surgery.

Home blood pressure diary
You can print the form off below to keep a record of your home blood pressure readings, one of the clinicians may request you to do this. Once completed, hand this in at reception and it will be reviewed by one of our clinical team.
